Colquitt County Schools offers voluntary Critical Illness coverage which provides a lump sum benefit in the event of a diagnosis of a covered illness. The plan is insured by MetLife and employees may elect coverage for yourself, your spouse, and your child(ren) with no health questions. (See certificate for complete details.)
Employees may elect in the amounts of $5,000, $10,000, $15,000, $20,000, $25,000, or $30,000. Employees may also elect to cover their spouses and / or child(ren). If dependent coverage is elected, the dependent(s) are covered at 50% of the employee amount.
Critical illness insurance helps you and your family maintain financial security during the recovery period of a serious medical event, such as cancer, heart attack, or stroke. The benefit provides a monetary benefit to help with the out-of-pocket medical and non-medical expenses of the illness, and coverage is available to both employees and their families.
Benefits are paid in addition to any other insurance that you may have, and benefits are paid directly to you
This plan pays a benefit if the initial diagnosis for a covered illness is while the certificate is in force. (See certificate for complete details.)

Benefits may be used however you'd like. Typical uses include:
Out-of-pocket medical and non-medical expenses
Home health care needs and home modifications
Recovery and rehabilitation
Child care or caregiver expenses
Travel expenses to and from treatment centers

Your Critical Illness certificate provides limited benefits – read your certificate carefully when you receive it. MetLife’s Group Critical Illness product is intended to supplement, and is not a substitute for, comprehensive medical coverage. By enrolling for Critical Illness Insurance, if I am a resident of Arkansas, Connecticut, Idaho, New Hampshire or Utah, I declare that no person proposed for Critical Illness coverage is covered under any Title XIX program (Medicaid or any similarly named program). I acknowledge that I have received a Shopper’s Guide to Cancer Insurance; and I have received and read a copy of the outline of coverage or other disclosure document for the group Critical Illness plan. In addition, I have read the enrollment documentation and declare that all information I have given is true and complete to the best of my knowledge and belief; I have read the applicable Fraud Warning(s) provided.
