Colquitt County Schools offers an accident plan through MetLife to protect you from unexpected accidents, as it can be difficult to financially plan for the unexpected. The MetLife Accident Plan provides financial protection in the event of an unexpected accident resulting in medical treatment. A summary of the benefits schedule is below. This plan reimburses you defined amounts towards your medical expenses and the benefit is paid directly to you. Coverage options are available for you, your spouse, and your dependent child(ren). It also includes a benefit for death, loss of limbs, hospital visits, and transportation by ambulance – as a result of a covered accident. Reference the plan summary below for more details.
Sports Accident Coverage
The MetLife Accident plan pays an additional 25% of the Accidental Injury, Accident - Medical Treatment and Services, Hospital Benefits, for accidents as a result of an organized sporting activity. Subject to limitations in the certificate.

Your Accident certificate provides limited benefits – read your certificate carefully when you receive it. MetLife’s Group Accident product is intended to supplement, and is not a substitute for, comprehensive medical coverage. By enrolling for Accident Insurance, if I am a resident of Minnesota or Vermont, I declare all persons to be insured have medical coverage in force that provides benefits for medical treatment, including hospital, surgical and medical expenses. I acknowledge that I have received and read a copy of the outline of coverage or other disclosure document for the Group Accident plan. In addition, I have read the enrollment documentation and declare that all information I have given is true and complete to the best of my knowledge and belief; I have read the applicable Fraud Warning(s) provided.
