Colquitt County Schools offers an accident plan through Prudential to provide financial protection against unforeseen accidents. Out-of-pocket expenses from an accident are often surprising. From deductibles and co-pays to additional everyday expenses, like childcare and food delivery, it may push you off track financially. Accident Insurance can help protect your budget and savings when you suffer a covered injury. It pays a benefit amount based on the type of injury and treatment you need. Payments are paid directly to you, and you can spend them however you wish. There’s no coordination of benefits with other coverages (such as medical), which means you’ll receive the full cash amount provided by your plan. 

Key Features:

Financial Protection: The Prudential Accident Plan reimburses defined amounts for medical expenses related to unexpected accidents, with benefits paid directly to you.

Coverage Options: Available for you, your spouse, and dependent children. It includes benefits for death, loss of limbs, and hospital visits due to a covered accident.

Sports Accident Coverage: The plan provides an extra 25% benefit for accidents resulting from organized sports activities. This includes Accidental Injury, Accident - Medical Treatment and Services, and Hospital Benefits. Please note, this is subject to limitations outlined in the certificate.

Wellness Benefit: The plan includes a wellness benefit covering preventive screenings for you, your spouse, and/or children, with a $50 reimbursement once per member per year.

 

How to File a Claim

  • Step 1: Visit www.prudential.com/mybenefits and login or register to complete your form electronically.
  • Step 2: Select “My claims” from the left side menu and click on the option to “File a claim/Report an absence.”
  • Step 3: Provide authorization for Prudential to obtain necessary medical information from your doctor, saving you time.
